Assessment FAQs

  • A copy of the Comprehensive Diagnostic Evaluation (CDE) for Medicaid
  • A copy of your insurance card (front and back)
  • A copy of your signed doctor referral with your child’s diagnosis and recommendation for ABA Therapy services.
  • A copy of your change in provider form (This is only needed if your child is currently receiving services from another company)
In order to complete the assessment, both the child and parent/guardian must be present.
It usually takes up to 1-2 hours for an assessment to be completed, depending on the circumstance of the child. In the event of a student absence, we will be able to provide a school absence note.
Each insurance company will have a different timeline for approval. The timeline can range from a few days to a few weeks, depending on the situation.
  • A request for authorization will be submitted to your current insurance provider once all required documents are received and completed.
  • An appointment will be scheduled within your respective clinic/area once your insurance approves the authorization request.
  • A Board Certified Behavior Analyst will conduct the assessment. Medical history, behavioral history, goals for your child, family goals, and other factors will be evaluated during the assessment.
  • Before we submit the assessment to your insurer, we will send you a copy for your electronic signature.
  • You will be contacted by our clinical team once your insurance has approved the assessment.

General FAQs

ABA is a type of therapy for autism that helps reinforce desired behaviors and discourage unwanted behavior. To do this, therapists use rewards to encourage communication, language, and other skills.

The type of ABA the patient receives will largely depend on their age and goals for treatment. ABA has proved successful at helping individuals with autism learn behaviors and skills, which has led many advocates to recommend it. This form of therapy is known as the “golden standard” for autism treatment. During regular evaluations, we can track your child’s progress. A consistent ABA therapy approach has been shown to increase developmental skills in children for the next stage of their lives.

At Synergy Behavioral we focus on clients under the age 21.
You will need to provide a doctor’s referral or prescription for us to perform an evaluation and treatment. If you are a private pay patient, you do not need a doctor’s referral.
